Sam, who was diagnosed with Ulcerative Colitis in 2003, has become an advocate for disability rights after her blog, So Bad Ass, has thousands of people.

Sam said: "Over the past three years I have worked so hard to raise awareness of the condition and to encourage others to seek help and to learn to accept their illness.

"I've shared my journey through surgeries and more and my aim has always been to help others.

“I hope in my role as Champion that I can continue on this path and be a help to people, their friends and families."
